Teacher Application for a Library Card

Library membership is free to all teachers, homeschooling parents and mentors in Knox and Waldo counties. Video/DVD and computer usage privileges must be completed and signed at the library. You must present your library card each time you check out materials. You may check out only materials to be used for your classroom - no materials for personal home use. If you would like to make personal recommendations for a student, we will put the book on hold and the student may check it out on his or her free card. You may request a special due date for items needed for a longer unit of study. We can give an additional two weeks in most cases.

If you have chosen to have your new card mailed to you, please allow 1 - 2 weeks for delivery.

If you have chosen to pick up your card at the library, you must provide proof of your current street address before you receive your card.

Patron card must be presented at the time of checkout of material. A service charge of $2.00 will be required to replace lost or stolen cards.

I accept responsibility for all fines incurred and for lost or damaged materials on this card. I agree to give immediate notice of any change of contact information or loss of my library card. I further agree to adhere to all policies of the library, and that any failure to comply with these rules may result in the revocation of my library privileges.
